Library Card Application


To borrow items, you need a valid TMU student card, TMU service card, TMU affiliate hospital’s service card or TMU library card. You can use one of these cards to:

  • enter the TMU Library
  • borrow items    
  • make reservations
  • use databases in the library

If you would like to apply for a TMU library card, you can visit the Information Desk on Mon-Fri from 8:30 - 21:45 and Sat-Sun 8:30 - 16:45. And follow the procedures according to your status:

  • TMU Student

    • You can use your TMU student card as a library card to borrow and access materials.
    • Period of Validity
      Your card is valid for the duration of your studies. After this, you can apply as an alumni member.

  • TMU Faculty / Staff

    • Who with a valid TMU service card.
    • Procedures:
      Complete a form to create your patron record at the library Information Desk.
    • Processing fee:
      Free.
    • Period of Validity:
      Your card will expire on the last day of your term of employment. After this, you can apply for a retiree membership or a TMU library member.

  • TMU Adjunct Faculty /Staff and Research Assistant

    • Who without a valid TMU service card.
    • Procedures:
      1. Download and fill out the application form.
      2. Visit the Information Desk with the following documents:

        ✚ 1-inch recent personal photo   
         Copy of your ID card
         Certificate of employment for reference
         ID card for reference
         Copy of your employment certificate

      3. After receiving a library notification, please visit library Information Desk to pick up your TMU library card.
    • Processing fee
      - Apply for a new card: Free
      - Replace a lost or stolen card: NT$50
    • Period of Validity
      Your card will expire on the last day of your term of employment.
      Please visit the library to return the library card or cancel your qualification before leaving TMU.

  • Affiliate Hospital Clinician and Staff

    • Who has a valid affiliate hospital’s service card.
    • Procedures:
      Complete a form to create your patron record at the library Information Desk.
    • Processing fee
      - Apply for a new card: Free
      - Replace a lost or stolen card: NT$50
    • Period of Validity:
      Your card will expire on the last day of your term of employment.
      Please visit the library to return the library card or cancel your qualification before leaving TMU.

  • Alumni

    • Procedures
      1. Download and fill out the application form.
      2. Visit the Information Desk with the following documents:

        ✚ 1-inch recent personal photo
        ✚ Copy of your graduate certificate (or certificate authorized by the TMU Alumni Liaison Office)
        ✚ Your graduate certificate or TMU alumni certificate

      3. After receiving a library notification, please visit library Information Desk to pick up your TMU library card.
    • Processing fee
      - Deposit: NT$3,000
      - Apply for a new card: NT$100
      - Replace a lost or stolen card: NT$50
    • Period of Validity: Lifetime validity

    • Important Note
      1. The TMU library card may be used for library access and borrowing TMU library materials only.
      2. You have to return all the books and pay all the funds before cancelling library borrowing praivileges.
      3. Please bring your library card and a copy of your remittance account, with the receipt of the original copy when canceling your TMU library card.
      4. The deposit is refundable when you cancel TMU library card without making payment of interest thereon. (Remittance crediting process takes about 2 months)

  • Emeritus Faculty and Retired Staff

    • Procedures
      1. Download and fill out the application form.
      2. Visit the Information Desk with the following documents:

        ✚ 1-inch recent personal photo
        ✚ Copy of your retirement certificate
        ✚ Retirement certificate

      3. After receiving a library notification, please visit library Information Desk to pick up your TMU library card.
    • Processing fee
      -Deposit: NT$3,000
      -Apply for a new card: NT$100
      -Replace a lost or stolen card: NT$50
    • Period of Validity: Lifetime validity
    • Important Note
      1. The TMU library card may be used for library access and borrowing TMU library materials only.
      2. You have to return all the books and pay all the funds before cancelling library borrowing privileges.
      3. Please bring your library card and a copy of your remittance account, with the receipt of the original copy when canceling your TMU library card.
      4. The deposit is refundable when you cancel TMU library card without making payment of interest thereon. (Remittance crediting process takes about 2 months)

  • Family of Current TMU/Affiliate Hospital Faculty and Staff

    • Procedures:
      1. Download and fill out the application form (Chinese only).
      2. Visit the Information Desk with the following documents:

        ✚ Applicant’s 1-inch recent personal photo
        ✚ Copy of applicant’s ID card
        ✚ Copy of staff member’s ID card or any documents that can prove your relationship with them (ex: household registry)
        ✚ Copy of staff member’s service card

      3. After receiving a library notification, please visit library Information Desk to pick up your TMU library card.
    • Processing fee
      - Apply for a new card: Free
      - Renew library card: Free
      - Replace a lost or stolen card: NT$50
    • Period of Validity
      Library cards expire every two years or expire on the last day of the staff member’s term of employment.

    • Important Note
      1. Age limit is 16+ (freshman of high school or older).
      2. Numbers of books borrowed will be counted under the staff’s record.
      3. Please confirm the borrowing privileges.

  • Visitor / TMU Library member

    • External visitors (at least 20 years of age) can enter the TMU Library by borrowing a temporary library card in exchange for your National ID card.
    • If you would like to eligible for borrowing privileges, please apply for a TMU Library member.
    • Procedures:
      1. Download and fill out the application form.
      2. Visit the Information Desk with the following documents:

        ✚ 1-inch recent personal photo
        ✚ Copy of your ID card
        ✚ ID card for reference

      3. After receiving a library notification, please visit library Information Desk to pick up your TMU Library card.
    • Processing fee
      - Deposit: NT$5,000
      - Annual fee: NT$2,000
      - Replace a lost or stolen card: NT$50
      - Cost of reproduction: NT$100
    • Period of Validity
      TMU Library member card is valid for one year upon the date of payment.

    • Important Note
      1. Temporary library card of TMU is for personal use only.
      2. Temporary library card is valid for up to one day. You have to return the temporary library card before leaving the TMU Library.
      3. The TMU Library card may be used for library access and borrowing TMU Library materials only.
      4. You have to return all the books and pay all the funds before cancelling library borrowing privileges.
      5. Please bring your library card and the receipt of the original copy when canceling your TMU Library card.
      6. The deposit is refundable when you cancel your TMU Library card without making payment of interest thereon. (Remittance crediting process takes about 2 months)
