Please follow the steps below (as shown in the screenshots):

1. Log in to the TMU Electronic Theses and Dissertations (ETD) system and go to Step 3: Print out and Upload the Authorization Form.


2. Click Preview and Print to double-check that all the information is correct.


3. Print one copy of the authorization form. After signing it, go back to Step 3 in the system.


4. Upload the signed authorization form to complete the process.
